Advanced Respiratory Disease & Proactive Planning Project (Leeds)

The healthcare and third sectors in Leeds are nationally recognised for innovation and accomplishment. Local Care Partnerships in the city are aligned with the Primary Care Networks and bring together all healthcare organisations plus social care and the third sector, enabling them to address the wider determinants of health in their local communities. Proactive Care, incorporating both personalised and anticipatory care, is being trialled in the Seacroft and Cross Gates areas of the city, with some very deprived parts. A data survey revealed respiratory disease as a major cause of anxiety, distress and hospitalisation. Care Coordination is bringing a Personalised Care approach to this patient cohort, as revealed in a video:
