“The 10 YHP sets a correct but ambitious objective for the NHS to take a prevention strategy, or “Shift Left” in the jargon. However, the system is currently almost 100% reactive by design. Given that, the fact that demand currently outstrips supply, that funding is essentially all allocated, and that management capacity is being halved, how can the NHS achieve this goal?

Well, the environment is certainly challenging (!) but the answer (as ever) lies in personalisation. This deck outlines:

  • how the system can start the process of inching towards a prevention model
  • shows the paucity of holistic support on offer against the prevailing need
  • builds on a pre-existing business case of the impact of supporting behaviour change
  • shows how digital can be a super-enabler for this work, opening up system-wide change, rather than offering support siloed in GP surgeries
  • proposes a solution that would offer a strategic backbone for neighbourhood models, and could fit well with the PHM contracts recently announced by DHSC/ NHSE as well as ICB or even regional commissioning
  • posits there are two fundamental issues which need addressing before this work can flourish: culture and funding

Personalised Care as a NHSE programme has disappeared. For those who believe in this way of working, this deck offers a chance to see the concept of personalisation take on a leading role in the development of a Neighbourhood health service fit for the 21st Century.”
