How to Make Neighbourhood Health Really Happen

“The Coalition for Personalised Care has co-developed with its members a practical means of using personalised care to help achieve the national three shifts: from disease to prevention, from hospital to community and from analogue to digital.

Our proposal complements the 10 Year Health Plan for England and the NHS Neighbourhood Health framework. It uses existing polices, flexibilities and procedures (so no need for primary legislation to make it happen) woven together in a novel way. Taken together with work already recommended within our Community Strengthening strategy it would help make health and care sustainable and would also lay the foundations for implementing a future National Care Service.

Putting these ideas into practice would be a radical change, but it’s entirely doable right now. The Comprehensive Model for Personalised Care is an enabler for this work. The thinking behind this proposal comes from work developed during the Personalised Care Programme (with particular thanks to colleagues from Cornwall, Juliet Ferris and Emma Rowse). The NHS nationally say personalised care is now “business as usual”, we know that’s not really the case in practice. This proposal would make it a reality.”
